Funding Application

Funding requests are reviewed annually by the Allocations Committee; typically during the month of April.  Both forms should be submitted electronically with the funding application that includes a logic model (and outcome measurements), financial statements, an audit and 990 (see application cover for a complete list).  New applicants are required to submit organization By Laws and Tax Exempt letter.  For more information, please contact the United Way office at (409) 835-4575.
